*Any girl in grades 6-8

*Any boy in grades 7-8

The Requirements:

*Permission Forms

*Photocopy of Insurance (health insurance card)

*Health History Form & Physical Form (combined in ONE file, choose Word or PDF format)

The Equipment:

*All athletes should have athletic sneakers that support the foot and ankle.
