The nurse’s of the Byram Township school district have made available many blank health forms and health information fact sheets.  These may be downloaded for your convenience.  PLEASE BE AWARE THE FORMS/INFORMATION MAY BE SPECIFIC TO EITHER THE BYRAM INTERMEDIATE SCHOOL OR THE BYRAM LAKES ELEMENTARY SCHOOL.

I  FORMS

 

A.  BYRAM INTERMEDIATE AND BYRAM LAKES ELEMENTARY SCHOOLS:

• Medication Policy - please note: for medication to be administered at school the following must be provided:

1. written statement (order) from the physician

2. pharmacy medication, in original box with current date.

3. parent/guardian signed authorization.

 

• Parent/Guardian Authorization Form - This provides the school nurse with parental/guardian permission to administer medication.

 

• Health Care Plan - Helps the school nurse to plan for the health needs of your child at school.  It must be reviewed and signed by your physician.

 

• Bus Driver Medical Need Notification Letter - If your child has an allergy or medical condition that you feel his/her bus driver should be made aware of, please take a few minutes to write the driver a note specific to your child’s needs.

 

B.    BYRAM LAKES ELEMENTARY SCHOOL:  Documents are Word Files.

• Physical Form - Required at entrance to school and recommended at each developmental stage as noted in the school calendar.
• Health Survey -  Parents opportunity to make school nurse aware of any pertinent health history of entering student.
• Registration Form - Required upon entrance to school and must be accompanied with:

*proof of residency

*original and copy of birth certificate

*official immunization history

 

C.     BYRAM INTERMEDIATE SCHOOL:

Sports Physical and Health History Form (New as of 8/27/10)           WORD   PDF	Sports Physical required for participation in sports and must be done within 365 days prior to the first practice by physician.
	Healthy History to be completed by parent/guardian within 60 days prior to the first practice. (and then signed off by physician)
Self Medication Form	This is for medications required in life threatening and emergency situations such as inhalers and epi pens.
Sports Activities and Medications	Students with a specific medical need including potential life threatening allergy or asthma, who want to participate in a school sponsored sport, must follow school medication policies and contact the school nurse regarding the health need.
